About Our Family Support
Partner Position:
Through the Child and Family Team
process, the Family Support Partner provides services in participant homes and
in the community, assisting families to navigate through multiple systems,
being an advocate, providing resources, and exploring natural supports.
The Family Support Partner will work with Child and Family teams and
collaborate with families, the Department of Child Safety, courts, and other
partners standing in for parents ensuring that the family's voice is
heard.
What a Family Support
Partner Does at Resilient Health:
- Provides
psycho-social rehabilitation services to youth with a diagnosis of a
serious emotional disturbance, co-occurring, or other behavioral health
concerns
- Provides home care
training, family services (family support) involving face-to-face
interactions with family member(s) directed toward restoration,
enhancement, and maintenance of family functioning, therefore, increasing
the family's ability to effectively interact and care for the person in
the home and community
- May involve support
activities such as assisting the family in adjusting to the child's
behavioral health needs, developing skills to effectively interact and/or
manage their child, understanding the causes and treatment of behavioral
health issues, understanding and effectively utilizing the system, and
planning their care. Teaches family to advocate on behalf of their child
- Teaches independent living,
social, and communication skills to the child and/or their families in
order to maximize the child's ability to live and participate in the
community and to function independently
- Provides ongoing family support
services that foster solution/strengths-based orientation to assist in
addressing the participant and family's ongoing needs
- Provide parenting support
through group and individual services
- Completes intakes (initial) and updated assessments
(including diagnosis) with participants and participating family members
or other supportive individuals
- Develops
individualized treatment plans in coordination with participants, their
families, and additional community members, as appropriate
- Assists
participants and their guardians in identifying and developing realistic
treatment goals
- Develops and
maintains effective working relationships with community agencies, case
managers, and clinical teams
- Maintains
appropriate clinical documentation and records accurately and timely, in
accordance with agency and funding source requirements
Requirements for This
Position:
- Must be, or have been, the parent/caregiver of a child with
behavioral health needs and have experience with children's service
systems (Behavioral Health, Education, Juvenile Justice, DFS,
Developmental Disabilities, etc.)
- High school diploma or equivalent required
- Behavioral
Health Technician required (must be 21 years old):
- High School Diploma or High School
Equivalency Diploma (GED) and six months of full-time behavioral health
work experience/internship or Peer Support Certification, or
- Associate degree and three months
of full-time behavioral health experience/internship or Peer Support
Certification, or
- BHT Certified from an accredited
college/university, or
- Bachelor's Degree, or
- Master's Degree
- Must have excellent verbal and
written communication skills
- Moderate typing, computer
processing, and technology skills required
- Must have skills necessary to
engage and work with others from diverse cultural groups
- Ability to provide your own reliable transportation required
- Ability to work 40 flexible hours a week including evening and
weekend availability required
- Valid driver's license
- Valid CPR/First Aid
certifications or ability to obtain within 90 days of hire
- Valid Fingerprint Clearance
Card or ability to obtain within 90 days of hire
- Three-year Motor Vehicle
