About our Targeted Case Manager:
Our Targeted Case Manager is responsible for delivering quality care to our participants through the coordination with a participant's recovery team in securing and managing general medical and behavioral health services that aid in the participant's recovery from mental illness and/or addiction. This position connects participants to community resources for social determinants of health.
What a Targeted Case Manager Does at Resilient Health:
- Works with other community resources in meeting/achieving consumer service needs
- Participates in the assessment, evaluation and planning of patient care
- Develops individualized service plans for participants
- Provides community and home based services to enhance and achieve treatment goals
- Develops, facilitates and coordinates meetings, staffings, case conferences and other meetings as appropriate
- Provides referrals to outside providers and assists in securing a variety of social services, including, but not limited to, relapse prevention services, medical care, and general assistance
- Maintains a minimum number of documented client contact hours per week
- Maintains appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements
- Communicates with the participant (or family member) to coordinate consent to care, releases and/or referrals for provider care, which includes behavioral care, general medical care, community resources, educational, social, judicial and other state agencies
- Coordinates transportation of participants to and from appointments when appropriate
- Consults with treatment staff to coordinate effectiveness of services contained in the participant's individual service plans and make adjustments where needed
- Provides outreach and follow-up on crisis contacts and missed appointments
- Meets with service providers to review utilization and monitor participant's progress towards program goals
- Provides participant advocacy and education as needed, to ensure delivery of required services
- Develops transitional service plan (including discharge planning) with participants, prior to reentry into the community
Requirements for This Position:
- High school diploma or equivalent required
- Behavioral Health Technician (BHT) required (must be 21 years old):
- High School Diploma or equivalent and six months of full-time behavioral health work experience or internship or Peer Support Certification, or
- Associates Degree and three months of full-time behavioral health experience or internship or Peer Support Certification, or
- A BHT Certificate from an accredited college/university, or
- Bachelor's Degree, or
- Master's Degree
- Valid Arizona or other state driver's license
- Valid automobile insurance
- Valid Fingerprint Clearance Card or ability to obtain within ninety days of hire
- Valid CPR/First Aid certifications or ability to obtain within ninety days of hire
- Three-year driving record that meets the company's auto insurance carrier's coverage requirements

What This Position Pays: $20.39 - $22.62, depending on experience and education
