About Our Nurse Practitioner
Position:
The Nurse Practitioner position entails
diagnosis, conducting assessments, and prescribing medications for patients
across the lifespan, including children and adolescents, having
psychiatric-mental health conditions, including those with serious mental illness
(SMI). The role also includes developing an appropriate, integrated healthcare
treatment plan and providing effective patient care management with proper
evaluation and follow-up care. This position will provide telemedicine
and in-person services based on patient and clinic needs.
What a Nurse Practitioner
Does at Resilient Health:
- Plan, organize, and provide ongoing
medication management for new and existing patients
- Conduct initial psychiatric evaluations
periodic medication management and monitoring visits
- Document patient visits in accordance with
agency standards
- Review diagnostic test results and
document the course of action taken
- Renew and refill medication to avoid
lapses in treatment
- Provide emergency face-to-face counseling
and/or crisis intervention as needed to avert, or in response to,
patient relapse
- Develop and maintain effective working
relationships with community agencies, case managers, primary care
providers, psychiatrists, and other members of the clinical team
- Participate in clinical staffing and
individual service planning sessions with case managers, treatment staff,
family members, and other stakeholders to identify and meet patient's
treatment needs
- Educate the patient in understanding the
disease process with guidance regarding preventative practices necessary
to manage acute and chronic conditions
- Order diagnostic tests and screening
evaluations
- Offer case consultation and clinical
training to Resilient team members
- Assist clients in identifying and pursuing
effective treatment goals
- Identify the types and amounts of support
clients require to remain independent despite acute and chronic
diseases
- Maintain information on community
resources for clients' assistance and support, with support from team
members
- Provide education, assistance, and support
to family members and others involved with the client to enhance awareness
of mental and emotional illness and to increase their ability to
contribute to the rehabilitation process.
Requirements for This
Position:
- Active Arizona nursing license required
- Advanced Practice Nurse Practitioner
Certification from AZ State Board of Nursing as a Psychiatric/Mental
Health Nurse Practitioner (across the lifespan) required
- Master's in PMHNP
- Experience in a behavioral health setting
preferred
- Bilingual in Spanish preferred
- Valid driver's license
- Valid CPR/First Aid certifications or
ability to obtain within 30 days of hire
- Valid Fingerprint Clearance Card or
ability to obtain within 90 days of hire
- Three-year Motor Vehicle Record that meets
our auto insurance carrier's coverage requirements

What This Position Pays: $150,000 - $160,000, depending on years of NP experience
