The Case Manager is responsible for the delivery of quality patient care to members via the coordination of a member's adult recovery team in securing and managing general medical and behavioral health services that aid in the member's recovery from mental illness and/or addiction.

The Outpatient Nurse Practitioner position entails diagnosis, conducting assessments, and prescribing medications for patients across the lifespan, including children and adolescents, having psychiatric-mental health conditions, including those with serious mental illness (SMI). The role also includes developing an appropriate, integrated healthcare treatment plan and providing effective patient care management with proper evaluation and follow-up care. This position will provide telemedicine and in-person services based on patient and clinic needs.
